Application for Tombstone Permit

NOTES

1. If the grave is that of a person who was entitled to the rights and priiviledges of the Burial Society of the Federation of Synangogues through membership, this application must be signed by a close family member of the deceased person. In any other case, the approval of the deceased’s legal representatives(s) must be obatained n the space provided below.

2. A list of monumental masons authorised to execute work of the Cemeteries of the Federation of Synangogues may be seen at (insert link?) the Office of the Burial Society. Applicants must not give an order to a mason not on the accredited list. This procedure is for the protection of the applicants themselves.

3. Officials and employees of the Burial Society are strictly forbidden from recommending directly or indirectly, the name of any particular mason, or from being concerned in any way with the placing of an order with a mason.

4. In accordance with Health & Safety regulations, if at any time the memorial becomes unsafe, the Federation Burial Society will make every effort to contact the Applicant named below who will be required to take prompt remedial steps to make the memorial safe. However, the Federation of Synagogues reserves the right at its discretion to take such action as it considers fit in order to make the memorial safe. In the event that such action is required the Applicant agrees to reimburse the Federation of Synagogues the cost of any restorative/remedial work involved.
